About the role

 

We are looking for an Evidence Access and Coordination Team Leader to join our team in the NDIS Section.

The NDIS Evidence, Access and Coordination (EAC) Team Leader will facilitate client eligibility for the NDIS and is responsible for coordinating access to the appropriate evidence informed assessment, referral to the NDIA and the development of a culturally appropriate plan. The EAC Team Leader will lead and work in a culturally responsive manner inclusive of the Aboriginal Community Connectors and the NDIS Aboriginal Family Support Worker, to ensure clients have the best outcome possible in accessing culturally appropriate NDIS plans. The EAC Team Leader will be highly functioning and able to support a team to produce desired outcomes.

The NDIS EAC Team Leader will ensure supporting and enhancing quality of life for clients in a range of settings to empower and assist Aboriginal people with disability, their families and communities. The NDIS program builds the capacity of NDIS clients to achieve greater independence to self-direct their required services and their own supports as a long-term goal. This position is a critical contributor to NDIS Access across Congress town and remote communities.
