About the role

 

The Child Health Outreach Program Nurse/AHP is integral to the Child Health Outreach Program working collaboratively with children 0 – 15 years and their families to facilitate and support children with complex chronic healthcare needs through coordination of care to optimise health outcomes.

 

The position has a strong clinical focus on outreach work including liaison with internal and external stakeholders involved in the child and family’s care.

 

Team Description

 

The Child Health Outreach Program uses a case management model of care to coordinate the care of Aboriginal children aged between 0-15 with chronic health conditions, and whose families social circumstances prevent them from meeting the health care needs of their child
