About the role The Child Health Outreach Program Nurse/AHP is integral to the Child Health Outreach Program working collaboratively with children 0 – 15 years and their families to facilitate and support children with complex chronic healthcare needs through coordination of care to optimise health outcomes. The position has a strong clinical focus on outreach work including liaison with internal and external stakeholders involved in the child and family’s care.
